Registered Nurses – Home Health Agency
To provide services to patients in accordance with the State Nurse Practice Act, agency, policy, and accepted professional standards of practice. Qualifications

Licensed Vocational Nurses – Hospice
To provide the following functions:
1. Provide quality nursing care and related services.
2. Maintains and demonstrates effective client services in an accurate and timely manner.

To perform skilled nursing functions in accordance with the federal, state, and local laws and within the guidelines of his/her professional organization and agency. Home Health Aide – Home Health Agency
To provide personal care services under the direction of the Registered Nurse or Therapist. The Home Health Aide is assigned to specific patients by the Registered Nurse or other appropriate professional and performs services for patients as necessary to maintain their personal comfort.
Qualifications
1. Successful completion of a formal certification training program and/or a written skills test and competency evaluation.
2. Be at least eighteen (18) years of age.
3. Minimum of six (6) months’ experience in a supervised setting, preferably in a health care facility.

Position Summary
Plans and provides physical therapy services to patients in their homes in accordance with the Plan of Care and agency policies and procedures.
