Registered Nurses – Home Health Agency

Licensed Vocational Nurses – Hospice
To provide the following functions:
1. Provide quality nursing care and related services.
2. Maintains and demonstrates effective client services in an accurate and timely manner.

Requirements

Home Health Aide – Home Health Agency
To provide personal care services under the direction of the Registered Nurse or Therapist. The Home Health Aide is assigned to specific patients by the Registered Nurse or other appropriate professional and performs services for patients as necessary to maintain their personal comfort.
Qualifications
1. Successful completion of a formal certification training program and/or a written skills test and competency evaluation.
2. Be at least eighteen (18) years of age.
3. Minimum of six (6) months’ experience in a supervised setting, preferably in a health care facility.

Physical Therapists
Position Summary
Plans and provides physical therapy services to patients in their homes in accordance with the Plan of Care and agency policies and procedures. The Physical Therapist performs functions which require substantial specialized knowledge, judgment, and skill based on the principles of psychological, biological, and social services and must be able to make sound judgment accordingly.
Qualifications
1. Graduate of a Physical Therapy program approved by the American Physical Therapy Association, the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council on Medical Education of the American Medical Association and the American Physical Therapy Association.
